Stabilization Model

Our 12-Month Stabilization Model

Our housing stabilization approach is built upon a structured, phased 12-month model designed to move residents from instability to independence.

Phase 1: Stabilization (0–3 Months)

The initial phase focuses on establishing stability and addressing immediate needs.

Key areas include:

  • Safe and stable housing placement
  • Orientation and program engagement
  • Identification and collection of required documentation
  • Benefits enrollment and verification
  • Workforce assessment and employment preparation
  • Establishment of individualized goals
  • Structured support and accountability systems

The objective of Phase 1 is to create a strong foundation for long-term success.
